Reevaluation Review Planning Meeting

Case Managers will conduct a review of the student's records and gather data through consultation with the student's stakeholders prior to the Reevaluation Review Planning Meeting. These stakeholders will include but are not limited to:

Following the review of the student's records and gathering of data in consultation with all stakeholders, the Case Manager will schedule a Reevaluation Review Planning Meeting . The Case Manager will ensure that at least two notifications of the meeting are provided to the parent. At least one must be the written Notice of Meeting.

Participants invited to the Reevaluation Review Planning Meeting are the required members of the IEP team, and other relevant members based on the student’s file review and consultations with the instructional stakeholders. If the Case Manager suspects that the team will propose gathering additional data through a formal evaluation, then the evaluator(s) (i.e. School Psychologist, Speech Therapist, OT/PT, Behavior Specialist, Vision or Hearing Teacher) should be notified prior to the meeting through the consultation process when completing the file review. This notification provides the evaluator(s) with the opportunity to attend the meeting and/or provide guidance regarding the potential evaluation request. In order to ensure that the reevaluation is completed in a timely manner, the Reevaluation Review Planning Meeting must occur four months prior to the triennial reevaluation due date.

At the Reevaluation Review Planning Meeting, the Case Manager facilitates the discussion of the student’s reevaluation by reviewing existing data about the student and identifying what additional data, if any, is needed to meet the educational needs of the student. The Reevaluation Review Planning Process is led by the case manager.

The team reviews the existing information for each area and determines whether additional information is needed for each area:
If the team determines that additional data is needed in any of the above areas, then the team determination is yes additional information is needed, and parent consent must be requested.
