Reevaluation Planning Meeting

Case Managers will conduct a review of the student's records and gather data through consultation with the student's stakeholders prior to the Reevaluation Planning Meeting. These stakeholders will include but are not limited to:

Previously this information was compiled and documented in the File Review process within EasyIEP. In Enrich IEP the information gathered should be documented within the Reevaluation Planning process.

Following the review of the student's records and gathering of data in consultation with all stakeholders, the Case Manager will schedule a Reevaluation Planning Meeting . The Case Manager will ensure that at least two notifications of the meeting are provided to the parent. At least one must be the written Notice of Meeting.

Participants invited to the Reevaluation Planning Meeting are the required members of the IEP team, and other relevant members based on the student’s file review and consultations with the instructional stakeholders. If the Case Manager predicts that the team will propose gathering additional data through a formal evaluation, then the evaluator(s) should be notified prior to the meeting through the consultation process when completing the file review. This notification provides the evaluator(s) with the opportunity to attend the meeting and/or provide guidance regarding the evaluation request. In order to ensure that the reevaluation is completed in a timely manner, the Reevaluation Planning Meeting must occur four months prior to the triennial reevaluation due date.

At the Reevaluation Planning Meeting, the Case Manager facilitates the discussion of the student’s reevaluation by reviewing existing data about the student and identifying what additional data, if any, is needed to meet the educational needs of the student. The Reevaluation Planning Process is completed by the case manager in Enrich IEP.

After reviewing the existing information for each area, the team determines whether additional data is needed in each area.
