Evaluation Planning

The Initial Referral Process begins with the Evaluation Planning Meeting. The purpose of this meeting is for school professionals and the parent to collaborate on the purpose of the evaluation and determine which areas will be assessed by the evaluator(s).


The Primary Evaluator will lead the team through the completion of the Evaluation Planning Meeting.

Once the Referral/Evaluation Planning Meeting document has been completed, the Primary Evaluator will print the Referral Evaluation Planning document for the Evaluation Planning Team to sign.


Participants of the Evaluation Planning Team will all sign the document and the Primary Evaluator will then upload and attach the signed document within the Referral/Evaluation Planning Process. In addition, the signed Notice of Meeting must be uploaded and attached within the Referral/Evaluation Planning Process.

Following the Referral/Evaluation Planning Meeting, the Primary Evaluator provides the parent with a Prior Written Notice (PWN) identifying the proposals and/or refusals related to “Evaluation/Reevaluation” and must clearly state why the evaluation planning team is proposing or refusing an evaluation. This PWN may be provided by the Primary Evaluator to the parent at the end of the Referral/Evaluation Planning Meeting, or may be sent to the parent following the meeting. Parent must receive the Prior Written Notice in advance of requesting consent to evaluate. The PWN is embedded within the Referral/Evaluation Planning Process and must be completed before the process is finalized.
