If the team determines that additional data is not needed, but new disabilities may be considered based on information provided by the parent from an outside agency (i.e. student is currently classified as Other Health Impaired and a new report indicates a medical diagnosis of Autism), the case manager should make sure that all necessary participants/evaluators are present at the Reevaluation Planning Meeting. Necessary participants would include an evaluator with expertise in the new disability category. For example, if a parent provides an outside agency report with a diagnosis of ADHD, Autism, Dyslexia, etc. invite the school psychologist to the meeting. Similarly, if an outside agency report is provided with diagnosis linked to orthopedic impairment, vision or hearing impairment the appropriate evaluator should be present. If they are not present, the meeting should be tabled until they can attend to address the new data.
If new disabilities are being considered (e.g. the parent providing information from an outside agency), and no additional evaluation is needed (e.g. the outside agency report is a comprehensive evaluation), : the Case Manager will complete the Reevaluation Planning meeting by printing the Reevaluation Review document the Reevaluation Planning Team to sign.
Participants of the Reevaluation Planning Team will sign the Reevaluation Review document and the Case Manager will then upload and attach the signed document within the Reevaluation Review process. In addition, the signed Notification of Meeting and outside agency report must be uploaded and attached within the process.
The evaluator then obtains the Consent for Reevaluation.
An Outcome Recommendation and End date must be entered to finalize the Reevaluation Review. Users will select "No Additional Information Needed - Eligibility Determination" as the Outcome Recommendation. The End Date should be the meeting date. Enrich IEP will generate a Pending Eligibility Determination.
The evaluator then leads the team through the Disability Worksheet(s) and the Eligibility Determination.
