reevaluation planning process questions

The Parent/Guardian/Age of Majority Student must be informed of his/her right to request an evaluation. After the parent(s)/guardian(s) are informed of their right to request an evaluation the Case Manager will complete the Reevaluation Planning meeting by printing the Reevaluation Review document for the Reevaluation Planning Team to sign. The Reevaluation Review document can be printed from the Documents section of the left-hand navigation menu.

Participants of the Reevaluation Planning Team will sign the document and the Case Manager will then upload the signed document within the Reevaluation Review process. In addition, the signed Notification of Meeting must be uploaded and attached within the Reevaluation Process

After completing the PWN, the Case Manager will select the Outcome for the Reevaluation Review process. If the Reevaluation Planning team determined no additional information is needed to determine eligibility the Case Manager should select “No Additional Information Needed - Eligibility Determination”, and select the meeting date as the End Date.

Enrich IEP will auto populate the student’s previously identified disabilities for consideration. Users should select “Yes” for the prompt “Disability Worksheet Needed?”. Selecting “Yes” will cause Enrich IEP to generate the disability consideration questions for the team to consider. Users may select “No” for the prompt “Enter Worksheet Data” for continuing categories of disability, but should select “Yes” for any new disabilities the team is considering.
