Consent to Evaluate
The Consent for Evaluation document can be accessed at the bottom of the Referral/Evaluation Planning Process. The Consent for Evaluation is completed by the Primary Evaluator during the Referral/Evaluation Planning Meeting as the team determines whether additional data is needed for each area.

Following completion, the Consent for Evaluation may be sent to the parent via one of the following methods:
- Provided in person
- Sent home with student
- U.S. mail
When the signed Consent for Evaluation is obtained from the parent, the Primary Evaluator completes the Outcome section of the Referral/Evaluation Planning Process indicating the Outcome Recommendation and the appropriate End Date:

- Consent Received - Evaluate: The End Date is the date the signed Consent for Evaluation was returned to the school.
- Consent Refused or No Response Received - PWN: If signed Consent to Evaluate was NOT received by the school within 30 days of the date sent, after at least two contact attempts using different contact methods, the Primary Evaluator will create a Prior Written Notice identifying the proposal to evaluate but that consent to evaluate was not granted by the parent. This PWN should list all contact attempts.
If Consent for Evaluation was granted by the parent, the Primary Evaluator will proceed with the evaluation. If the parent does not give consent for the evaluation, the Primary Evaluator completes a Prior Written Notice. The purpose of this PWN is to clearly state that the parent has refused consent for the proposed evaluation, and to explain the school’s proposed next steps.
The signed Consent for Evaluation will be uploaded within the Referral/Evaluation Planning Process in Enrich IEP prior to finalizing.
