Required Forms

We are no longer accepting paper copies of physicals. Please follow the directions below.

Print this Physical Form (PDF) and take it to a doctor for a physical exam. Please make sure your doctor signs and dates the physical. All lines must be filled in, including athletes’ date of birth. Once the physical is complete, the athlete and the parent will need to create an account with Arbiter Sports.

We have simplified the registration process, and all waivers are now signed digitally! Please be sure to follow these steps to complete your registration and ensure that your student is registered for athletic participation:

1. Create an Arbiter Registration account. You only need one account for your whole family.

2. Activate your Arbiter Registration account from the activation link that will be automatically sent to your email. (Note: The email may go into your junk folder.) If you are having trouble creating your account, receiving the activation email, or logging in, you may contact support by email at or call 800-311-4060.

3. Once you have successfully created your account and are logged in, you must complete your registration on our Mauldin Middle School Arbiter Sports Landing Page:

4. You will receive an email notification to let you know when we have received your registration. Students must be registered and cleared to be eligible for participation.

*Physicals must be dated after April 1 and are good through the end of that school year. If an athlete turns 15 years of age before July 1 of the upcoming school year, they are too old to play middle school sports and must play at the high school.

Academic Requirements

Middle School Academic Requirements apply to students enrolled in the 7th and 8th grade wishing to try out for sports.

Fall & Winter Sports

Students must have passed the four required core courses (ELA, Math, Science, and Social Studies) the previous school year and been promoted from the previous grade to be eligible to participate in Fall and Winter sports.

Spring Sports

Students must be meeting the school district promotion policy—passing 4 core courses—at the end of the first semester in order to be eligible for sports during the second semester.

Any student ineligible due to not passing a course at the end of the first semester may try out for a spring team if their third quarter grade is high enough to pull their overall grade to passing (meeting the district promotion policy), and if there is space available on the team.

Exception: Students taking Algebra 1 or English 1 and failing the course. This is due to the fact that these are advances courses, and they may take the courses again as a 9th grader. Regardless, the student must be promoted into the next grade.
