Required Forms

We are no longer accepting paper copies of physicals. Please follow the directions below.

Print this Physical Form (PDF) and take it to a doctor for a physical exam. Please make sure your doctor signs and dates the physical. All lines must be filled in, including athletes’ date of birth. Once the physical is complete, the athlete and the parent will need to create an account with Planet HS.

Use your smartphone and send a text to 69274 with the message S1552. If you do not have smartphone, visit Planet HS and make sure to put Mauldin Middle as one of your schools.

Step 1: Student creates an account. Create a username and password and set Mauldin Middle as the home school. 

Step 2: Parent creates an account. This must be a separate account from student, including using a separate email and phone number.

Step 3: Student “links” the parent account. This is similar to sending a friend request on Facebook. Use either the phone number or email that the parent used to create their account.

Step 4: Student fills out the Concussion Form.

Step 5: Parent fills out all six forms. Steadman Hawkins is not optional. The doctor page of the Physical Form and the student's birth certificate must be uploaded. (The parent must do these two forms in particular because there is a line for them to sign and put the date of the physical.) Using a smartphone, take and upload pictures of the Physical Form and birth certificate to the parent account (from mobile site). You may also choose to scan and upload copies of these documents.

Once all paperwork is submitted, it will say “Pending staff approval”.  When approved, an email confirmation will be sent to both the student and parent accounts.

*Physicals must be dated after April 1 and are good until June 30. If an athlete turns 15 years of age before July 1 of the upcoming school year, they are too old to play middle school sports and must play at the high school.

Academic Requirements

Middle School Academic Requirements apply to students enrolled in the 7th and 8th grade wishing to try out for sports.

Fall & Winter Sports

Students must have passed the four required core courses (ELA, Math, Science, and Social Studies) the previous school year and been promoted from the previous grade to be eligible to participate in Fall and Winter sports.

Spring Sports

Students must be meeting the school district promotion policy—passing 4 core courses—at the end of the first semester in order to be eligible for sports during the second semester.

Any student ineligible due to not passing a course at the end of the first semester may try out for a spring team if their third quarter grade is high enough to pull their overall grade to passing (meeting the district promotion policy), and if there is space available on the team.

Exception: Students taking Algebra 1 or English 1 and failing the course. This is due to the fact that these are advances courses, and they may take the courses again as a 9th grader. Regardless, the student must be promoted into the next grade.
