GREENVILLE COUNTY SCHOOLS MEDICATION POLICY

All medication must be delivered to the School Nurse or other designated school employee by the student’s parent, legal guardian or a previously designated adult. It is against School District Policy for a student to carry ANY medication on their person without physician permission, parent permission and school district permission to self-administer and self-monitor. (SC Law S144) All medication will be maintained in the health room. Violations of the medication policy are dealt with sternly. Medication needed on field trips is also covered by this policy and must be given to and administered by an employee of Greenville County Schools designated by the principal. There are separate permission forms for medications on field trips.


Prescription medication:
1.  All PRESCRIPTION MEDICATIONS must have written physician and parent/guardian signatures on Prescription Medication Form.  (FORM MED 1 HERE)
2.  Medication must be in the original container, and have PHARMACY LABEL, with correct administration instructions.
3. Students are not allowed to deliver/carry medication to school.
4.  Expired medication will not be accepted.
5.  Parent/Guardian is responsible for knowing the expiration date of any medication brought to school, and replacing the medication before the expiration date.  Nurses will not administer expired medications.
6.  If the student will be carrying EMERGENCY medication (inhaler, EPI PEN) the Authorization for Self-Administration form must be completed and signed by MD and parent/guardian. (FORM MED 2 HERE)  Student agreement to self-administer form (FORM HERE)

Over the counter medication:
1.  Parent/Guardian must complete NON PRESCRIPTION medication form. (FORM MED 3 HERE)
2.  Medication must be in original container.
3.  Students are not allowed to deliver/carry medication to school.
4.  Medication will be administered per label instructions.  Over the counter medications may not exceed label directions.
5.  Expired medication will not be accepted.
6.  Parent/Guardian is responsible for knowing the expiration date of any medication brought to school, and replacing the medication before the expiration date.  Nurses will not administer expired medications.
7.  No medication containing aspirin will be administered.
