Physicals: Hello!
We will plan to do Sports Physicals for Volleyball next Wednesday, August 19, 2020 from 9:00am-12:00pm, in the health room. Attached is the consent packet along with the Sports Physical Form that will need to be completed and returned to the school by 12:00pm Tuesday (English and Spanish version). I will come to the school Tuesday afternoon to pick up the returned completed packets and will call parents to give them a specific time to bring their student on Wednesday morning- so that we can maintain proper social distancing.
Because we need to review the health history of each student beforehand, we will not be able to perform physicals on any student that does not return their paper work by the deadline on Tuesday.
All students trying out must have a current physical, parent consent packet forms completed and turned in, and insurance (can be bought through the school) is also mandatory.
