SOUTH CAROLINA IMMUNIZATION REQUIREMENTS

PLEASE READ VERY CAREFULLY

South Carolina Law requires that all students have a South Carolina Certificate of Immunization Form on file at the school. Your child will be allowed to attend school for 30 days on a TEMPORARY CERTIFICATE. During this 30 day period, it is the parent's responsibility to make sure the school has the proper certificate. PLEASE BE AWARE THAT YOUR CHILD WILL NOT BE ALLOWED TO ATTEND SCHOOL WITHOUT THIS CERTIFICATE. The minimum requirements are:

• 3 doses of DTP vaccine with at least one received on or after the 4th birthday.

• 3 doses of oral or inactivated Polio vaccine with at least one on or after the 4th birthday.

• 1 dose of MMR (Measles, Mumps, and Rubella) and a second dose of Rubeola both after the 1st birthday and separated by at least one month.

IF YOUR CHILD WILL BE ENTERING THE 7TH GRADE HE/SHE MUST HAVE STARTED THE HEPATITIS B SERIES (3 SHOTS)

PLEASE CHECK THE STATEMENT BELOW THAT APPLIES TO YOUR CHILD:

_____    I am bringing a South Carolina Immunization Certificate with me. I realize that my child must have
               a second measles vaccination and if he/she is entering the 7th grade must also have started the
               Hepatitis B series. If this is not documented on my present Immunization Certificate, I must get a new
               certificate.

_____    I am transferring from a public school in Greenville County. I have a transfer paper indicating that an
               immunization certificate is on file and will be sent with my child's record. I realize that my child must
               have a second measles vaccination and if he/she is entering the 7th grade must also have started the
               Hepatitis B series. If this is not documented on my present Immunization Certificate, I must get a new
               certificate.

_____    I am moving from another district in South Carolina. I understand I have 30 days during which the
               school must receive a copy of my child's certificate. I realize that my child must have a second
               measles vaccination and if he/she is entering the 7th grade must also have started the Hepatitis B
               series. If this is not documented on my present Immunization Certificate, I must get a new certificate.

_____    I am moving from out of state. I understand I have 30 days to secure a South Carolina immunization
               certificate and bring a copy to the school. I realize that my child must have a second measles
               vaccination and if he/she is entering the 7th grade must also have started the Hepatitis B series.
