  • Leave Request Form - Form used to request a Leave of Absence. Complete and submit to your supervisor
  • Certification of Health Care Provider - Employee - Employee Illness- Use this form to document intermittent or longer illness (i.e. planned surgery, maternity, etc). This form needs to be completed by your physician and returned with Leave Request Form.
  • Certification of Health Care - Family Member - Family Member - Use this form to document intermittent or longer illness (i.e. planned surgery, maternity, etc) for a family member. This form needs to be completed by the physician.
  • FMLA Military - Certification of qualifying Exigency for Military Family Leave
  • FMLA Veteran - Certification for Serious Injury or Illness of a Veteran for Wage and Hour Division Military Caregiver Leave
  • FMLA Injury Illness Military - Certification for serious injury or illness of covered Service member for Military Family Leave
  • Return to Work Form - Complete this form and present to your supervisor with documentation from your physician that you are released to return to work (see leave process for details).
