Dear Parents and/or Caregivers,

Welcome to the Southside Health Room Parent page. I will be using this page to share with the parents’ information that you may need to be aware of. I encourage you to take a look at the Student page so you can be aware of what is expected of your student. My home page gives you my background information. I will list below some of the things we need at school to better serve your student. I have added the Health Room Forms that are most often used. You are welcome to download and print these forms from your home.

 We do not supply any medication for any reason. You must send a permission slip and a small bottle of that medication to school. I will accept your child bringing it directly to the Health Room before the start of the school day. If your child does not bring it before school they will have to answer the consequences if they get caught carrying it on them. I will share the medication with brothers and sisters, but will need separate permission slips
 If your doctor prescribes a medication that must be taken during school hours, I will need a signed Doctors order as well as a Permission slip from you.
 I encourage those students that need an inhaler to carry it with them at all times. Please send the required Doctors order as well as your permission slip.
 I encourage all my female students to bring in the medication they routinely use during monthly cycles. Feminine products are available in the Health Room. The fee for each is .25.
 If your student has any medical condition that the nurse should know about please notify me as soon as possible. Please fill out the Student Emergency Form and return it to the Health Room. Even if your student does not have any medical conditions, I need to know your wishes in the event of illness or injury.
 I do close the health room at 200 PM, except for emergencies. This allows me time to return phone calls, have conferences, or catch up on paper work. If you have left a message for me to contact you I will call you back after 200 PM, unless it is an emergency.
 If you ever have any questions about your student, yourself or another family member, please feel free to contact me. I will be happy to answer any questions or point you in the right direction to get them answered.
