On the first day of school, your child will be given a Student Emergency Information Form. Please print all information clearly. Notify the school nurse with any changes throughout the school year.
Students in grades K,1,3,and 5 will be screened for distant vision problems. Students in grades K,1,2 and 3 will be screened for hearing problems. If your child is not in one of these grades and you want to have them screened, please send a note.
All medications must be maintained in the Health Room. You may contact the school nurse or visit the GCSD Health Services website for information or to obtain authorization forms.
Students are required to have a valid SC Certificate of Immunization or a medical, religious or special exemption.
