Health Forms

All students are required to have a Student Emergency Form. Forms are also available for the administration of medications, students with severe allergies and students whom are asthmatics.

Physician’s Authorization
Required for prescription medications given at school for more than 2 weeks
Parent Permission
Required for ALL medications given at school
Student Emergency Form
Required for ALL students. To be updated yearly.
Allergy Emergency Form
Required for students who experience severe allergic reactions
Asthma Care Plan
Required for students with asthma who use inhalers
