A Note from the Nurse

Dear Brushy Creek Families.. Welcome back to the 2011-2012 school year.  I am looking forward to seeing all of you on the first day of school.  Parents- please remember if your child needs to have a prescription medication given at school that a Doctor and Parent Authorization form must be completed before the medication can be kept and/or used at school. No medications will be accepted without these forms per Greenville County School District Guidelines.  PLEASE REMEMBER STUDENTS ARE NOT PERMITTED TO CARRY MEDICINE TO SCHOOL PER GREENVILLE COUNTY SCHOOLS GUIDELINES. If you need my assistance please leave a message for me at 355-5407 and I will be glad to help you. Thanks and see you soon!  ~~Ms. Davies, RN- Brushy Creek Elementary School

Following District Policy, this year, health rooms will not be able to hold or dispense any prescription medications including inhalers and epi-pens without a Physician/parent authorization form. This authorization form MUST be provided by the parent on the FIRST day the medication is brought to the school for use.  Medications CANNOT be held or dispensed without this form. Forms are available in the Front Office at any time and below:

Written parental authorization must be provided that includes the student's name, dosage, time medication is to be given and the duration of administration.
