Dear Brushy Creek Families.. Welcome back to the 2011-2012 school year. I am looking forward to seeing all of you on the first day of school. Parents- please remember if your child needs to have a prescription medication given at school that a Doctor and Parent Authorization form must be completed before the medication can be kept and/or used at school. No medications will be accepted without these forms per Greenville County School District Guidelines. PLEASE REMEMBER STUDENTS ARE NOT PERMITTED TO CARRY MEDICINE TO SCHOOL PER GREENVILLE COUNTY SCHOOLS GUIDELINES. Following District Policy, this
year, health rooms will not be able to hold or dispense any
prescription medications including inhalers and epi-pens
without a Physician/parent authorization form. This
authorization form MUST be provided by the parent on the
FIRST day the medication is brought to the school for use.
Medications CANNOT be held or dispensed without this form.

District Rules for Medications:
All Medication must be brought to the
health room by a parent in the
original container, clearly labeled
with the student's name.
The first dose of any medication
must be given at home. The parent
must observe the student for at
least two hours.
Written permission from the parent
or legal guardian for each
medication is required and will be
kept in the health room.
A written
Physician's authorization must be
provided for any medication given
for more than 10 consecutive school
days. We have the forms in the
health room. This is needed for
inhalers you would like to have kept
in the health room to be used as
needed also (see next).

All prescription medication must be
delivered to the school by a parent,
guardian, or person over 18 years of
age designated by the parent and
given directly to the school nurse
or school staff member.
A parent may supply an
over-the-counter medication to be
kept in the Health Room and
administered by the nurse. Written
parental authorization must be
provided that includes the student's
name, dosage, time medication is to
be given and the duration of
administration. The medication must
be in its original container,
clearly labeled with the student's
name.
We cannot administer anything that
is not FDA approved. Most herbal
remedies fall into this category.
